Use the federal directory to reach your state agency. Ask about vision eligibility, participating providers, eye exams, prescription glasses, replacements, costs and authorization rules. Adult benefits vary; do not assume every service is covered.
U.S. applicants with household net income below 250% of poverty guidelines may qualify. A prescription from the last 24 months with pupillary distance is required. The voucher covers basic glasses through its dispenser, not an eye exam. A nonrefundable application fee and optional upgrade charges apply; confirm amounts before applying.
Ask a local club or district whether it currently helps with exams or glasses. Assistance depends on the local program and available funds. A recycling donation location is not automatically an assistance provider.
New gift-certificate distribution to partners is currently paused according to VSP. Existing certificates and mobile-clinic events have separate instructions. Do not count on a new voucher until the program confirms availability.
An eye-exam program may not pay for glasses. The National Eye Institute lists programs by age and need, insurance routes and local referrals. Check the provider’s current rules before applying.
